For New Patients

Prior to beginning treatment with Atlanta Area Psychological Associates, P.C., patients are required to complete several forms. To save time on your initial visit, you may download and complete these forms prior to visiting our office. Please bring the completed forms with you on your first visit.

Insurance Information

We are contracted with most of the major insurance companies. We also accept Georgia Medicaid for patients up to age 21. We are also on two of the managed Medicaid plans, Peach State (to age 21) and Amerigroup (all ages). We accept Medicare, Tricare, Aetna, Blue Cross Blue Shield, Cigna, Magellan, Multi-plan, United Healthcare (UBH/Optum) and Carelon. While not all providers are on all insurance plans, our intake coordinator will assist you with finding a provider that is contracted with your insurance company.
